Primary Care Providers in Canadian, Texas
Canadian Family Physicians is a primary care clinic dedicated to providing acute, chronic, and preventative care services of the highest professional standards in a respectful and friendly environment. We offer individualized care to each patient, diagnose and treat common medical conditions, and even perform minor procedures in the clinic. Our team of primary care providers is experienced in serving rural communities and takes pride in providing compassionate and quality medical care to patients of all ages, from birth to seniors. We particularly enjoy treating families as a whole—the true essence of Family Medicine.
Our goal is to partner with our patients to better address all their healthcare needs. We encourage patients to ask us questions, and we always take the time to listen carefully and respond to their concerns. While we offer comprehensive primary care services within our clinic next to Hemphill County Hospital, we will refer you to other specialists if and when needed.

What is Primary Care?
“Primary care” is your first stop for medical care, commonly referred to as your healthcare home. Primary care providers (PCPs) are trained to prevent, diagnose, and treat conditions you may have throughout your life span.
Typically, if you need general care, check-ups, or have any type of health concern, you can seek treatment from a primary care provider. In some cases, your PCP may refer you to a specialist for more specific treatment, depending upon your needs and condition.

What is Preventative Care?
Preventative healthcare includes annual check-ups, age-appropriate screenings, and patient education and counseling to detect possible health-related problems early when they are most treatable and to prevent unforeseen illness or disease in the future. Routine preventative care can lead to improved quality of life and ultimately, increased life expectancy.
Our mission is to empower patients to take an active role in their care plan by partnering with their primary care provider. By listening to, informing, and involving patients in their care, we can support our patients in making the most effective healthcare decisions that will result in better health and a heightened sense of well-being. Our staff puts the needs of patients first in order to help them achieve their health goals. This philosophy is practiced regularly by Canadian Family Physicians. Some notable examples include:
Same day/next two days appointments so that patients are able to see a provider when needed.
CFP's dedicated primary care providers (PCPs) make every attempt to have each patient see their preferred PCP. In the event that a particular provider is not available due to scheduling, another one of our providers will be available.
Secure email allows a convenient way for patients to stay in touch with their PCP and the medical staff. Our providers appreciate updates regarding the health of their patients, and offer assistance to help maneuver the complicated medical landscape.
Our clinic typically runs on schedule, and we make every effort to ensure that we are ready to see you at or near your appointment time. Occasionally, there are circumstances that are beyond our control which delay us, but we have constructed our system and workflow to make your time in our office as productive and useful as possible.

G.A. "Tony" Cook, MD
Family Medicine
Dr. Tony Cook grew up on his family’s ranch in Hemphill County and graduated from Canadian High School in 1992. After receiving a Bachelor of Science in Animal Science from Texas Tech University in 1997, he proceeded to University of Texas Southwestern Medical School in Dallas, graduating in 2002. Dr. Cook then spent three years in residency training for Family Medicine at Texas Tech University Health Sciences Center in Lubbock after which he returned to his home town of Canadian, Texas, and co-founded Canadian Family Physicians in 2005. Despite his professional name, Dr. Cook has always been known in Canadian as Tony (from his middle name, Anthony).
Dr. Cook is Board Certified in Family Medicine and is passionate about providing exceptional healthcare for patients throughout the HCHD region. As a primary care provider at Canadian Family Physicians, Dr. Cook has a full-time practice treating patients of all ages, from newborns to seniors. In addition to his practice at CFP, he also provides ER coverage and cares for inpatients at Hemphill County Hospital. He especially enjoys performing procedures in his practice, including skin cancer and skin lesion removal, joint injections, and more in clinic, as well as GI endoscopy (colonoscopy and EGD) procedures in Hemphill County Hospital’s GI Lab. Dr. Cook also serves as the Chief of Staff at Hemphill County Hospital and as Medical Director of Mesa View Senior Living, HCHD’s long-term care community.
Dr. Cook is very involved in his home town, and he is passionate about investing in Canadian’s future generations. He very much enjoys teaching gun safety, marksmanship, and sportsmanship as a Hemphill County 4-H Shooting Sports Instructor in shotgun, rifle, and pistol. Away from work, Dr. Cook enjoys hunting, fishing, scuba diving, and just playing around on the ranch with his family. His wife, Wendie, is the director of the Citadelle Art Foundation, and they have three boys.

Cody G. Culwell, MD
Family Medicine
Dr. Cody Culwell grew up on his family’s farm in Swisher County and graduated from Tulia High School in 1994. He then received his Bachelor of Arts degree in Biology from Baylor University in 1998, graduating Summa Cum Laude. From there he attended UT Southwestern Medical School in Dallas, graduating in 2002. Dr. Culwell then completed his family medicine residency at Texas Tech University Health Sciences Center in Lubbock and was named resident of the year for 2004-05. He then returned to his hometown of Tulia to practice for four years. Dr. Culwell then jumped at the opportunity to move to Canadian in June of 2009 to join Canadian Family Physicians with Dr. Tony Cook.
Dr. Culwell is Board Certified in Family Medicine. In his full-time practice, he enjoys caring for patients of all ages and has interests in sports medicine and minor procedures including skin lesion removal. He also provides ER and inpatient care at Hemphill County Hospital. He is medical director for Hemphill County Home Care Services including home health and hospice. Furthermore, he serves as medical director for both Hemphill County and Gray County EMS services.
Dr. Culwell and his wife, Melissa, have six kids including three who have already graduated from Canadian High School. He enjoys spending time with his family and sports in his free time. In addition, he has a passion for missions, having gone on several mission trips in the past few years. He currently serves as a deacon in his church, and he is actively involved in the community, serving on the Canadian ISD board of trustees since 2012.

Anna Isaacs, MD
Family Medicine
A native of Northern Minnesota, Dr. Anna Isaacs is a 2010 honors graduate from Montana State University, where she majored in Finance, ran collegiately for two years, and spent most of her free time downhill skiing or hiking. Dr. Isaacs returned to her home state to attend the University of Minnesota Medical School, receiving her Doctorate of Medicine in May, 2015.
Dr. Isaacs completed her three-years of family medicine residency at John Peter Smith Hospital (JPS) in August, 2018. The largest family medicine residency program in the country, JPS also ranks in the top five nationwide. Dr. Isaacs was elected by her peers to serve as Chief Resident in 2017-2018. Upon conclusion of her residency, Dr. Isaacs received her Board Certification in Family Medicine and joined Canadian Family Physicians on December 3, 2018. She provides a full scope of family medicine services in her practice, and she’s passionate about comprehensive women’s healthcare, pediatrics, lactation support, and mental health. Dr. Isaacs loves caring for all age groups, from infants to the elderly.
She is married to local native Tom Isaacs. Because of Tom’s love of the family ranch, which has been in the family since the late 1800s, it only seemed right to begin building her profession and start a family in Canadian. Dr. Isaacs and Tom have three children.

Tiffany Potter, FNP-C
Family Medicine
Tiffany Potter, FNP-C, grew up and graduated high school in Pampa Texas. She completed her RN training at Amarillo College, graduating in 1995. She worked as a medical-surgical and OR nurse for two years. Following, Tiffany had the honor to provide care to terminally ill hospice patients for 13 years in the eastern Texas panhandle.
Feeling that she could provide a more in-depth level of service to patient care, Tiffany returned to college, graduating from Texas Tech University in 2010 with a Bachelor of Science and in 2012 with a Master of Science in Nursing. Tiffany began her FNP career providing services in acute medical rehab and long-term care facilities in Amarillo. In 2013, she was made aware of the need for service to populations in rural communities and began to provide care to the underserved, indigent, and immigrant populations in Moore County; further developing experience with the complexity of care in rural areas of Texas.
Tiffany joined Canadian Family Physicians in July, 2014 with 20 years of experience in the medical field. In addition to providing primary care services at CFP, Tiffany trained under Dr. Briant Herzog at Hormone Therapy Centers of America (HTCA) in Plano, and she continues to provide bio-identical hormone replacement therapy in her practice. Tiffany is dedicated to offering the safest and most effective healthcare options to her patients.
Tiffany has two adult daughters. Her hobbies include Special Olympics, DIY projects, traveling, and visiting historical sites. Tiffany and her daughters attend Trinity Fellowship Church in Amarillo, serving faithfully for over 20 years.

Beverly Stephenson, FNP-C
Family Medicine
Beverly Stephenson, FNP-C was born and raised in Canadian and graduated Canadian High School in 2002. She attended Oklahoma State University in Stillwater and received a bachelor’s degree in Biological Sciences in 2006. After working in veterinary pharmaceuticals for three years, she then proceeded to study nursing at West Texas A&M University (WTAMU) in Canyon, graduating Sigma Theta Tau with a Bachelor of Science in Nursing in 2010.
Beverly worked as an RN for six years, first in medical-surgical and ICU in Pampa, then transferred to labor and delivery at Northwest Texas Hospital in Amarillo. She then enrolled in WTAMU’s Family Nurse Practitioner program and received a Master of Science in Nursing in 2017. Beverly practiced in women's health and primary care at a gynecology clinic in Amarillo for four years, where she gained valuable experience in the gynecological needs of women throughout their lifetime, as well as increased knowledge of the wide range of routine and preventative healthcare services provided in primary care.
On November 1, 2021, Beverly returned “Home” to Canadian and joined the team at Canadian Family Physicians. In addition to serving the health needs of families in our community, Beverly thoroughly enjoys the practice of women’s health and providing care to women of all ages.
Moving back to Canadian has allowed Beverly the opportunity to raise her children in her own hometown. In her spare time, Beverly enjoys the outdoors—riding horses, checking cattle, fishing, and camping. She is married to Bryan Stephenson who is also an outdoorsman and native Canadian, Texan.

Katelyn Booze
Clinic Manager
Katelyn Booze was born and raised in White Deer, Texas. She attended West Texas A&M University where she completed her Bachelor of Science in Nursing in May, 2013 and began working as an RN in June, 2013. Katelyn joined the staff at Hemphill County Hospital in the spring of 2014, working in the Medical/Surgical Unit and Emergency Room. She then transitioned into the role of Director of Nursing, where she served for five years.
In the summer of 2020, Katelyn completed her Master’s in Healthcare Administration at Texas Tech University Health Sciences Center. Katelyn accepted the position of Rural Health Clinic Practice Manager for the Hemphill County Hospital District in the spring of 2021, overseeing HCHD’s local rural health clinic, Canadian Family Physicians, as well as Harvester Family Medical Clinic in Pampa, Texas. In addition, she oversees HCHD’s behavioral health clinic, Hope Awakened Counseling, which provides professional counseling services to residents in Canadian and surrounding communities. Katelyn believes in empowering staff to reach their greatest potential while serving the health needs of rural Texans.
Katelyn married her college sweetheart, Jake, in the fall of 2014. In her spare time, Katelyn enjoys spending time with her husband and two children, being outdoors, playing golf, helping care for the horses and cattle, and watching sunsets from her front porch.
