Physical Therapy
HCHD’s licensed physical therapists (PTs) and physical therapist assistants (PTAs) are professionally trained in helping patients reduce pain, improve or restore mobility, recover from injury or surgery, and so much more. We provide care for people in a variety of settings, including treating patients at our outpatient therapy center, our hospital’s inpatients, and patients of home health, as well as residents in our assisted living and senior living communities.
Physical therapists utilize treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. PTs can teach patients how to prevent re-injuries or manage their condition so that they will achieve long-term health benefits. In addition, PTs work with individuals to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.

At the onset of a person’s first visit, a licensed physical therapist will evaluate the patient’s medical and physical history, the injury or disorder, age, level of activity, and other lifestyle factors in order to develop the most effective customized treatment plan unique to that individual. Just as no two patients are exactly the same, we believe no two treatment plans should be exactly the same.
Physical Therapy services are available with or without orders from a physician—no referral required. To get started, simply call our office to schedule an evaluation.

Meet Our Physical Therapists
Justin Frazier, DPT
Justin Frazier is a Doctor of Physical Therapy specializing in outpatient orthopedic and sports rehabilitation. He joined HCHD’s Therapy Services team in April 2026 after relocating to the region.
Justin earned his bachelor’s degree in Exercise Science from the University of Texas at Austin and completed his Doctor of Physical Therapy degree from the University of Texas Health Science Center at San Antonio. Since graduating, Justin has worked in a variety of physical therapy settings, with the majority of his experience in outpatient orthopedics and sports performance environments, including working with high school, collegiate, and several professional athletes.
In his approach to patient care, Justin enjoys helping individuals build a deeper understanding of their bodies and movement. He believes that physical therapy goes beyond simply “getting better,” and instead focuses on empowering patients to develop autonomy and confidence in how they move and function. Justin especially enjoys working with young athletes, as he values sports as a powerful platform for developing character, discipline, and life-skills.
Justin holds certifications in dry needling, along with multiple advanced private certifications in athletic rehabilitation and sports performance protocols. His training allows him to take a comprehensive, performance-driven approach to rehabilitation—helping patients both recover from injury and develop a stronger and more capable movement foundation.

Justin moved to the area from Fort Worth in 2026 with his wife and their two young children. Outside of the clinic, he enjoys spending time with his family—whether that’s having dance parties, playing at the park, or building rock towers.
CARA SMITH, DPT
Cara Smith is a Doctor of Physical Therapy specializing in outpatient orthopedic and sports rehabilitation. She joined HCHD’s Therapy Services team in July 2026 after relocating to Canadian.
Cara earned her bachelor’s degree in biology from Lubbock Christian University and completed her Doctor of Physical Therapy degree at Texas Tech University Health Sciences Center. She brings a decade of experience working in an outpatient orthopedic clinic in Lubbock.
With a passion for helping patients recover from injuries, restore movement, and build healthy habits that support long-term wellness, Cara enjoys working with patients of all ages and believes physical therapy should be both challenging and enjoyable. Her goal is to create personalized treatment plans that help each patient gain confidence, improve function, and return to the activities they love. She is certified in dry needling and orthopedic manual therapy for the spine.

Cara moved to Canadian in 2026 with her husband and three kids. She enjoys spending time with her family, being outdoors, and participating in church activities.
Janna Markham, PT
Janna Markham, PT has been a physical therapist at Hemphill County Hospital District since 2010. She became interested in physical therapy in elementary school while attending physical therapy sessions with her dad. As she entered high school, she decided to pursue physical therapy as a career, hoping to provide access to physical therapy services in rural communities.
After graduating from Reydon High School in Reydon, Oklahoma, Janna attended Southwestern Oklahoma State University in Weatherford and received her Physical Therapy degree from the University of Oklahoma Health Sciences Center in 2000. Janna began her physical therapy career providing therapy to students in several small schools in Oklahoma. She then worked for a rehab company providing physical therapy to patients at skilled nursing units throughout western Oklahoma for approximately 10 years—and this is where she developed her love for working with elderly patients. Janna joined the staff at HCHD in 2010 where she provides therapy services to patients in HCHD’s Swing Bed program and home health, as well as to long-term care residents at Mesa View Senior Living.
Janna enjoys caring for patients of all ages, but she truly loves working in home health and with the geriatric population. The home health setting gives her the ability to work on specific areas patients need in order to live independently in their own homes. It is very rewarding to Janna when she helps patients return to their favorite activities and improve their quality of life.
Janna lives with her husband, Monty, and two sons on a ranch in southeast Hemphill County where they raise cattle. Spending time with her family and supporting her boys in their many sports and activities brings her the most joy. Janna also volunteers with the Roger Mills County 4-H Adult Council, serves on the board of the Cheyenne-Reydon All Sports Association, and is a member of the American Legion Auxiliary.
Trint Flowers, PTA
Trint Flowers is a Physical Therapist Assistant. After graduating from Murray State College in Tishomingo, Oklahoma, he worked with the geriatric population in home health, swing bed, and skilled nursing facilities across Southeastern Oklahoma for six years. Trint joined HCHD in January of 2023 and is a proud member of the Therapy Services team.
In caring for patients, Trint strives to get them focused on the betterment of their own personal health through achievable, rewarding goals. He believes therapy should be challenging, fun, and geared toward improving overall quality of life.
"Physical therapy is the most fulfilling career path I could have chosen. Seeing patients’ faces light up when they see their own progress and hard work paying off is something you don’t get to see in very many jobs. We are going to celebrate and cheer on all the progress and achievements made—no matter how big or small."
Trint lives with his wife, Jasmine, and three children in Canadian. He enjoys spending time with family, playing music, and being active through exercise and various sports.
Occupational Therapy
Occupational therapy is a client-centered health profession concerned with promoting health and well-being through occupation. The primary goal of occupational therapy is to enable people to participate in the activities of everyday life. Occupational therapists achieve this outcome by working with people and communities to enhance their ability to engage in the occupations they want to, need to, or are expected to do, or by modifying the occupation or the environment to better support their occupational engagement.
It is the only profession that helps people across the lifespan do the things they want and need to do through the therapeutic use of daily activities (occupations). Occupational therapy practitioners enable people of all ages to live life to its fullest by helping them promote health and prevent—or live better with—injury, illness, or disability.
Common occupational therapy interventions include helping children with disabilities to participate fully in school and social situations, helping people recovering from injury to regain skills, and providing supports for older adults experiencing physical and cognitive changes. The profession also focuses strongly on providing independence in activities of daily living which include bathing, dressing, toileting, feeding, grooming, and personal hygiene.
Occupational therapy practitioners have a holistic perspective, in which the focus is on adapting the environment and/or task to fit the patient, and the patient is an integral part of the therapy team by helping establish treatment goals and interventions. It is an evidence-based practice deeply rooted in science. Occupational therapists not only look at the physical aspects of their patients but also at their mental, emotional, and psychosocial well-being. We truly help you live your best life at home, at work, and everywhere else (American Occupational Therapy Association, 2022).

Meet our Occupational Therapist
KEAGHAN HOLT, OTD, OTR
DIRECTOR OF THERAPY SERVICES
Dr. Keaghan Holt is a 2019 graduate of Seguin High School and went on to graduate Cum Laude from Angelo State University with a Bachelor of Science in Health Science Professions in May 2022. She continued her education at Texas Tech University Health Sciences Center, where she proudly earned her Doctor of Occupational Therapy degree in May 2025.
During clinical rotations, Keaghan gained experience in outpatient pediatrics and inpatient rehabilitation. Keaghan completed her capstone in school-based occupational therapy. She has since worked in outpatient pediatrics and skilled nursing as a travel occupational therapist. These experiences further strengthened her passion for serving patients across a variety of practice settings.
Dr. Keaghan Holt joined our Therapy Services team in January 2026. She provides therapy services to HCHD’s swing bed program, outpatient adult and pediatric patients, residents at the Mesa View Assisted Living and Mesa View Senior Living communities, and home health patients. In July 2026, Keaghan was promoted to assume the position of Director of HCHD Therapy Services.
Keaghan came to Canadian with a passion for rural healthcare and caring for patients across the lifespan. She enjoys the unique challenges of practicing in multiple settings and strives to provide engaging, patient-centered care—supporting individuals with compassion through some of life’s most challenging moments. She is excited to continue growing as both a clinician and a leader alongside such a supportive team, administration, and community.
Keaghan lives with her fiancé, Lucas, in Perryton. Outside of the office, she enjoys traveling, spending time with her family, and relaxing with her cats, Raider and Red. She also loves a little friendly competition and can often be found playing trivia at The Stumblin’ Goat Saloon.

HCHD’s Speech Language Pathologists hold a Master of Science degree in Communication Disorders and can assess, diagnose, and treat disorders related to speech, language, cognitive-communication, voice, swallowing, and fluency. Meet our Speech Therapists
MARY GRACE RAYBURN, M.S., CCC-SLP
Mary Grace Rayburn is a Speech Language Pathologist and joined Hemphill County Hospital District in the summer of 2023. Mary Grace is a Pampa native and after graduating from Pampa High School in 2000, she attended Rhodes College in Memphis, Tennessee for her undergraduate degree, which she completed in 2004.
After seeing her dearly loved, World War II hero of a grandfather who suffered from Parkinson’s Disease be treated with kindness, compassion, and respect for his individual personality and needs by a Speech Language Pathologist, Mary Grace immediately knew this was the field for her to pursue. She then graduated from the University of Texas at Dallas for her Master of Science degree in Communication Disorders. She has worked in radiology for swallow imaging, acute care, outpatient therapy, home health, skilled nursing, and long-term care.
She strives to treat each patient with personalized care for their speech, communication, and swallowing needs. Eating together and communicating are the things that make up a great life—and Mary Grace’s passion is to help plan and assist patients to be able to live life to the fullest. She feels incredibly grateful to have the privilege to be a part of her patients’ lives and counts it all joy to get to walk through difficulties with them, rehabilitating people to a fulfilling, engaging life of eating meals together and communicating with those around them.
Mary Grace lives in Pampa with her husband and three kids. They love any sort of competition and being involved with their family and church body.
PARKER LEE, M.S., CF-SLP
Parker Lee is a Speech-Language Pathologist and joined Hemphill County Hospital District Therapy Services in June 2026. She earned her Master of Science degree in Speech-Language Pathology from Texas Tech University Health Sciences Center in 2026.
Originally from Canadian, Texas, Parker graduated from Canadian High School in 2021 before attending Texas Tech, where she received her Bachelor of Science degree in Speech, Language, and Hearing Sciences in 2024. Through her academic experiences and clinical training, she discovered a passion for helping individuals improve their communication, swallowing, and overall quality of life. Her dedication to the profession grew through the meaningful relationships she built with patients and families throughout her education.
Parker completed a clinical rotation in Canadian, serving residents at Mesa View Senior Living and patients at Hemphill County Hospital. She began her professional career with HCHD Therapy Services, providing services across outpatient, home health, long-term care, and acute care settings. Parker is committed to delivering patient-centered care by meeting each individual where they are, tailoring treatment to their unique needs, and empowering them to achieve their personal goals. She strives to equip patients with the tools and strategies needed to communicate effectively, maintain safe swallowing, and continue participating in the activities they enjoy.
Parker enjoys staying active, spending time outdoors, and making memories with friends, family, and her dogs. She loves all things outdoors and values quality time with the people she cares about most.
Respiratory Therapy
Hemphill County Hospital District has introduced a new line of service designed to support individuals living with chronic lung conditions—our Respiratory Therapy Program. Offered in partnership with Meridian Healthcare Group, this service brings advanced respiratory expertise directly to our community.
Meridian has been “Shaping the Future of Respiratory Care” for over four decades. As one of the largest providers of respiratory care in the United States, it has built a national network of clinical programs in hospitals, skilled nursing facilities, outpatient clinics, home care centers, and sleep labs.
The program is staffed by Respiratory Care Practitioners who provide pulmonary exercise, psychological support, and education on managing chronic lung disease. Working alongside referring physicians, RCPs develop personalized plans to strengthen breathing muscles, improve daily function, and enhance overall quality of life.
Respiratory therapy has been shown to reduce breathlessness, increase stamina, and significantly improve health for individuals with COPD, emphysema, chronic bronchitis, pulmonary fibrosis, and other chronic lung diseases.

Meet our Respiratory Therapist
BAILEY HUDSON, RRT
Bailey Hudson is a Registered Respiratory Therapist with more than ten years of experience providing respiratory care to patients of all ages with both acute and chronic respiratory conditions. Patient care has always been a driving passion for her, and she is committed to supporting the health and well-being of the communities she serves.
Bailey grew up in Hereford, Texas, and graduated from Hereford High School in 2012. She then moved to Amarillo to pursue her education at Amarillo College, where she completed the Respiratory Care Program in 2015.
She spent the next decade working at a hospital in Amarillo, gaining extensive experience across a variety of respiratory care settings. During this time, she cared for patients with a wide range of respiratory conditions and developed a strong foundation in both critical and routine respiratory care.
While working full-time, Bailey continued her education at West Texas A&M University and earned a BBA with a specialization in Healthcare Administration in 2019. She also worked part-time at Amarillo College, assisting with the Respiratory Care Program and supporting the education of future respiratory therapists.
During the COVID-19 pandemic, Bailey accepted a travel assignment at the University of New Mexico Hospital in Albuquerque, a Level I trauma center. She spent six months there caring for critically ill patients and supporting healthcare teams during an exceptionally challenging period in healthcare.
Although she lived and worked in Amarillo and Hereford for many years, Bailey has deep family roots in Canadian, Texas. Many of her family members have been lifelong residents, and she has remained closely connected to the community throughout her life. In May 2025, she and her husband—a Canadian High School alum—moved to Canadian to raise their family. They have a three-year-old son who keeps them entertained and on their toes. In their free time, they enjoy spending time outdoors, traveling, and being with family and friends.
Bailey became part of Hemphill County Hospital District’s new Respiratory Therapy Program when it launched in April 2026. She looks forward to supporting the program’s continued growth and providing high quality respiratory care to its patients. Bailey is proud to be part of HCHD and is excited to serve the Canadian community.
